Emergencies

If you have a life threatening emergency please call 911.  For all other emergencies call our Elgin office at 847-742-9698 or our Algonquin office at 847-458-0173.  If it is after office hours call our answering service at 847-931-6546.

Sick Appointments

Please call our office after 8:30 AM to schedule your child’s sick appointment.  Sick children will be seen in our offices the same day.

Well Care

Well child appointments, school, and sports physicals are scheduled in advance.  Every effort is made to accommodate urgent physical and immunization appointment requests. 

We schedule well care appointments from birth to 3 years of age at the recommended schedule, which we will provide to you.  We recommend annual well care appointments for all children 3 years and above. 

Asthma, Allergy, Attention Deficit Hyperactivity Disorder (ADHD), & Children With Special Needs

If your child has one of the above conditions, we will make every effort to schedule your appointment as soon as possible.

Please fill out the form below to submit non urgent requests.  DO NOT SUBMIT ANY APPOINTMENT REQUESTS.  ALL APPOINTMENT REQUESTS NEED TO BE MADE BY CALLING OUR OFFICE.  If you submit a request and do not hear back from our office within 24 hours, please call our office.
