Account Information

Each patient is given an account number. It is important that you have that account number with you all of the time. When you call our office, please give the account number so that the receptionist can help you. If there is a change in your insurance plan, please inform us as soon as possible. If you have any questions on your account, please contact the accounts supervisor.

For new patients, you should give the following information:

Child's name
Guarantor's name:
Address
Phone number
Child's birthdate
Insurance Plan

• Policy #
• Group #
• Copay $ (if applicable)
• Deductible (if applicable)

If HMO The name of the HMO and the IPA that you belong to. It will be your responsibility to be on our eligibility list for the HM0.

You should also give us the important medical information about your child and bring a copy of their previous immunizations
